Your Academic Transcript must be a certified copy (i.e. a copy of your academic transcript that has been certified to be a copy of the original by a certifying professional, such as your local pharmacist).
Please collate the pages from an academic transcripts into a single PDF file; a simple online 'pdf-combiner' can be used to join scanned pages together.
Please ensure all file-names start with your first and last name, followed by their description.

All upload-items specified on the below upload-check-lists are MANDATORY, except for:

(1) photo/s of a lockable filing cabinet or draw, for those who do not keep paper-based records,
(2) photo/s of a client waiting area for those who do not have a waiting area - [in the absence of a client waiting area, AARPN assumes and stipulates that all consultations are staggered, so as to avoid client overlap], and
(3) photo/s of Capillary Sampling Guidelines in consultation area, for those who do not engage in finger-prick capillary sampling [relevant poster guideline for this can be found on our website alongside the other required posters].
